2011-2012 Back To School Information
Dear CMS Families,
Welcome to the 2011-2012 School Year! We have just finished our first week of school for our returning students. As we have done in years previous to ensure a smooth transition into the school year, start dates for new students will be staggered and will begin next week.
This year we are going green! We will not be using Friday folders. All information will be sent to you electrically via email or it will be posted to our website. You are welcome to send a tote bag in with your child to bring home their finsihed work. However please remember we are about the process and not the product. So do not be alarmed if your child is not bringing home a lot of papers.
Families who have chosen to pay tuition monthly will find a document outlining the process to register for FACTS included in the Back to School information. Please be sure to enroll or update your information ASAP. The first tuition withdrawal begins in August on either the 5th or 20th.
Below are the links for paper work for each program. Items that must be printed, filled out and returned to the school are marked so. Please bring your forms to either the Parent Information Night or the Open House. We MUST have these items on your child's first day!
Emergency Contact/Car Pool (Must be filled out and returned)
Health Record/Immunizations (Immunization portion must be filled out and returned)
Student Release of Information Form (Must be filled out and returned)
Toddler IN Ministry Form (Must be filled out and returned for all Toddler students)
Elementary "Goings Out" Form (Must be filled out and returned for all Elementary students)
Volunteer Criminal History Check (Must be filled out and returned for any family member who would like to volunteer at the school)
